About the Program

Integrated Post-Acute Orthopedics is a physician-led, non-interventional orthopedic service line delivered on-site in skilled nursing facilities across the greater Jacksonville area. We manage the orthopedic recovery of post-acute patients — after hip fracture, joint replacement, spine surgery, fracture fixation, and amputation — as well as non-surgical musculoskeletal conditions like osteoarthritis, rotator cuff disease, compression fractures, and the MSK complications of stroke and deconditioning, as well as support long-term care residents in their ongoing care.

We don't perform injections, procedures, or surgery, and we don't prescribe narcotics. Our value comes from diagnostic precision, protocol-driven non-operative management, systematic surgeon coordination, and evidence-based bracing, DME, and rehab direction. Our philosophy is function-first: restore pre-morbid function, prevent readmission and secondary fracture, and return each patient to their highest achievable baseline.

We work as true partners to facility medical staff, therapy teams, nursing, and the operating surgeons who entrust us with their patients' recovery.

The Role

We are seeking a licensed Physician Assistant or Nurse Practitioner with orthopedic experience to evaluate and manage post-acute orthopedic patients at partner facilities in the Jacksonville area. You will practice at the top of your license with defined clinical protocols, physician backup, and a systematized escalation pathway — independent in your daily workflow, never unsupported.

What You'll Do

  • Perform bedside evaluations of post-operative and non-surgical orthopedic and musculoskeletal conditions
  • Provide post-operative surveillance — incision and wound monitoring, DVT/PE vigilance, and procedure-specific red-flag recognition for arthroplasty, spine, and fracture-fixation patients
  • Escalate to the operating surgeon and SNF primary care using defined criteria and a clear communication pathway — surgeon handoff is a responsibility we take seriously, not a courtesy
  • Direct therapy plans, bracing and orthotic recommendations (unloader braces, AFOs, splinting), and imaging within SNF coverage realities
  • Remove sutures and staples and perform basic orthopedic wound care
  • Build bone health into every fragility fracture recovery — osteoporosis evaluation, bisphosphonate stewardship, and secondary fracture prevention before discharge
  • Manage post-stroke musculoskeletal sequelae — shoulder subluxation prevention, spasticity-phase bracing, gait and fall-risk management
  • Collaborate daily with facility physicians, APRNs, therapists, and nursing staff
  • Document to the standard that drives post-acute care: notes that support skilled need, Medicare Advantage continued authorization, and compliant Part B billing

What We're Looking For

  • Active Florida licensure (PA, NP, or MD/DO) in good standing
  • Minimum 2 years of orthopedic experience (inpatient, outpatient, or post-acute setting)
  • Strong musculoskeletal exam and clinical reasoning skills — differentiating operative from non-operative problems is the core of this job
  • Comfort working independently across SNF/ALF environments, with geriatric experience strongly preferred
  • Excellent communication and documentation skills — our notes are read by surgeons, facility teams, and payers
  • EMR proficiency (Elation Health or similar a plus)
  • Reliable transportation and ability to travel between facilities in the greater Jacksonville area
